Subcontractor Safety Prequalification

All subcontractors must complete a safety prequalification prior to issuance of any contract for service by August Winter & Sons, Inc. Prequalifications must be completed annually.

What You'll Need

Before starting the form, please have the following information ready:

Have These Ready

✓ Company information (address, phone, EIN/Federal Tax ID)
✓ OSHA 300 Log data for the last 3 years
✓ Experience Modification Rate (EMR) for the last 3 years
✓ Number of annual hours worked for the last 3 years
✓ Safety / Health professional contact information
✓ Your August Winter vendor number, if you have one (check your POs)

How It Works

Step 1: Complete the Form

Fill out the online prequalification form with your company and safety information.

Step 2: Submit

Your information is securely submitted to our Safety Department for review.

Step 3: Download Your Copy

After submitting, download a PDF copy of your prequalification for your records.
